SUMMARY
Introduction:
Nasal extranodal T/NK (ENKEL-NT) lymphoma is a rare and aggressive form of non-Hodgkin lymphoma (NHL) associated with Epstein-Barr virus (EBV) infection. It is characterized by neoplastic proliferation of the T/NK cell lymphoreticular system. Its incidence varies, being common in regions such as Asia and Latin America.
Clinical case:
A 30-year-old male patient with no significant medical or family history, employed as a call center operator, consulted for unintentional weight loss of 10 kg associated with a feeling of mass, obstruction in the right nostril, and cacosmia. Imaging studies were performed, revealing a mass involving the midline, subcutaneous tissue, gums, and hard palate. A biopsy of the mass was obtained, revealing atypical lymphoid proliferation and necrotic material, and immunohistochemistry was positive for CD56, CD4, CD2, and CD8, confirming nasal ENKEL-NK. EBV was confirmed by serology and viral load.
Discussion:
ENKEL-NT is an unusual tumor, and given its wide range of differential diagnoses, it can be difficult to diagnose accurately. Due to its location, it can progress indolently and self-limit to a disseminated form that can impact prognosis and management. Its definitive diagnosis is limited to histopathology and in situ hybridization for EBV.
Conclusion:
ENKEL-NT is an unusual and aggressive form of NHL, posing a diagnostic challenge due to its wide differential margin. Early histopathological diagnosis could have an impact on disease control and survival.
