Local consolidative therapy (LCT) following treatment with nivolumab (Opdivo) and ipilimumab (Yervoy) did not enhance survival rates in patients diagnosed with metastatic non-small cell lung cancer (NSCLC), particularly in those with oligometastatic disease.
Study Findings
In the phase III LONESTAR trial, the median overall survival (OS) was 43.2 months when LCT was added to nivolumab-ipilimumab, compared to 52.8 months with nivolumab-ipilimumab alone (HR 1.14, 95% CI 0.75-1.74, P=0.54), as reported by Dr. Mehmet Altan from MD Anderson Cancer Center during the World Conference on Lung Cancer (WCLC) held in Seoul, South Korea.
A negative trend in OS was also noted among patients with oligometastatic disease in the LCT-dual immunotherapy group, with a median OS of 42.0 months versus 75.8 months in the nivolumab-ipilimumab group alone (HR 1.68, 95% CI 0.87-3.26, P=0.12).
No Improvement in Survival Metrics
The study did not demonstrate a statistically significant enhancement in progression-free survival (PFS) either. For all patients, median PFS was recorded as 31.3 months in the LCT-dual immunotherapy arm versus 24.3 months in the nivolumab-ipilimumab alone arm (HR 0.79, 95% CI 0.54-1.15, P=0.22). In patients with oligometastatic disease, median PFS was 35.7 months compared to 44.0 months, respectively (HR 1.38, 95% CI 0.76-2.52, P=0.284).
