Introducción
The FDA has expanded the approval for teplizumab (Tzield), a biologic agent designed to delay the progression of preclinical type 1 diabetes to its symptomatic form, now including children as young as 1 year old, as announced by Sanofi on Wednesday.
Picos de Diagnóstico
The initial and most common peak for type 1 diabetes diagnosis in children occurs between the ages of 4 and 7, with a second peak between ages 10 to 14. Previously, teplizumab was approved for children aged 8 years and older.
Importancia de la Aprobación
This approval represents a significant development in diabetes care for young children at stage 2 of type 1 diabetes and their families. According to Kimber Simmons, MD, MS, from the Barbara Davis Center in Aurora, Colorado, this is crucial as these children are at a higher risk of rapid progression without warning.
Delaying the onset of stage 3 during these years, when management is often more challenging due to the child’s size and dependence on caregivers, could have a profound impact on families.
Stages de la Diabetes Tipo 1
Type 1 diabetes progresses through four stages. At stage 2, patients are still presymptomatic but exhibit dysglycemia and at least two related autoantibodies. Stage 3 is recognized as the clinical stage, where significant beta cell destruction occurs, necessitating insulin for hyperglycemia management.
Mecanismo de Acción
Teplizumab is a CD3-directed monoclonal antibody that works by deactivating immune cells that attack insulin-producing cells, while enhancing the proportion of cells that help regulate the immune response.
Resultados del Estudio PETITE-T1D
The PETITE-T1D study provided crucial support for this approval. This open-label trial involved 23 patients with stage 2 type 1 diabetes, administering a daily intravenous infusion for 14 consecutive days. The average age of participants was 4.8 years.
An interim analysis estimated a 89.6% probability of not progressing to stage 3 at 1-year follow-up, with only two participants moving to stage 3. Serum teplizumab concentrations peaked on day 14 of treatment.
Seguridad y Efectos Adversos
No new safety risks were identified in the study, and most adverse events reported were mild to moderate in severity. However, three participants discontinued treatment due to anemia, elevated liver enzymes, and maculo-papular rash.
This study also noted that 78.3% of participants experienced gastrointestinal disorders and infections, along with other side effects including vomiting (52.2%), rash (43.5%), and diarrhea (30.4%).
Advertencias y Precauciones
While the drug has no absolute contraindications, it is important to consider warnings regarding cytokine release syndrome, serious infections, lymphopenia, hypersensitivity reactions, and the timing of vaccinations. A boxed warning highlights the risk for viral reactivation.
Sanofi also mentioned that the FDA is reviewing the drug’s capability to delay further progression of type 1 diabetes in patients aged 8 years and older who are already in stage 3.
